The following information must be submitted to the National Safety Council to apply for status as a First Aid Training Center or Instructor.
Each First Aid Training Center and Instructor Agreement submitted is carefully and individually reviewed. There is one training center agreement and you indicate if you wish to teach the general public or privately to your employees and/or their families.

Please sign and date the training center agreement and check the courses you wish to teach.
Submit the completed agreement with copies of current instructor credentials (i.e. AHA, ARC, medic first aid instructor/trainer credentials, RN, first responder, EMT or paramedic certificate, etc.) or use the new online grandfathering application! This interactive tool will allow new instructors, who are qualified to grandfather in, the ability to submit their application and credentials through an online form. Simply fill out the First Aid Training Center/Instructor E-Application and upload your credentials. If you do not qualify to grandfather in you may enroll in an instructor course.
Upon approval of the instructor credentials, each instructor must purchase an instructor resource kit, which includes a student manual, instructor manual, DVD, and an instructor CD-ROM.
This application must be submitted by all prospective instructors and training centers regardless of the preferred method of payment for materials.
