Affinity Risk Control Awards Program

Affinity Insurance LTD and the National Safety Council would like to honor you for keeping your employees safe.

Please complete the following information to receive your award(s).

The Perfect Record Award honors members that have achieved 12 or more consecutive months without an incident requiring a day away from work

The Million Work Hours Award honors members that have achieved more than 1,000,000 consecutive hours with an incident requiring a day away from work

Affinity members are only eligible for one each of the awards per calendar year. The Affinity/NSC awards program is for the company as a whole, so only one "Affinity/NSC" award will be delivered for each member company (if meeting the criteria). If a major operation (i.e. distribution center) meets the criteria but the rest of the company does not, the member will not be eligible to receive the award via the Affinity program but is welcome to participate in the NSC Workplace Awards Program by visiting nsc.org/memberawards.
