How to Analyze Incidents Without Blame
1 p.m. (ET) Oct. 14

Incidents are caused by miscommunication, misunderstanding and other complex errors. How do you learn from them to prevent similar events in the future? This webinar will discuss how simple techniques like the "five whys" often fix the blame but don't fix the problem, how root cause analyses are often too complex and expensive, and how learning teams are popular but have unique drawbacks. Join Jake Mazulewicz, Ph.D., director of JMA Huma Reliability Strategies, LLC, for this fast-paced and engaging webinar. You’ll see plenty of real-world examples and learn how to apply the event review process to incidents in your industry.
