Honors companies, units or facilities that have achieved 10 consecutive "Perfect Record" years by operating at least 120 consecutive months without incurring an occupational injury or illness resulting in days away from work.

Distinguish your organization, or a division within your company, for completing at least 12 consecutive months without an occupational injury or illness involving days away from work.

This award recognizes the companies, and divisions within a company, that have accumulated at least 1 million work hours without incurring time away from work due to occupational illness or injury.

Honors companies that have had a successful hazard recognition program in place for the past four or more years.

Honors companies, locations or divisions that have achieved at least five consecutive years (60 months) without incurring an occupational injury or illness.

In accordance with OSHA record-keeping requirements, honors companies, locations or divisions that reported injuries and illnesses including days away from work equal to or less than 50% of the BLS rating for the 6-digit NAICS code, and have had no fatalities during the calendar year.

Honors companies, locations or divisions that have had at least 30 consecutive days without an occupational injury or illness involving days away from work.
